Community Care Programs

Concord Primary Care Services, Inc., a State of Texas licensed Home Health Agency is certified to provide the following Texas Department of Aging and Disability Services (DADS) community care programs to assist individuals to return or remain in the community through individualized service plans consisting of the following services:

ADULT FOSTER CARE

A 24-hour living arrangement, which includes supervision by a trained AFC provider. Services are provided in private residential settings that house no more than one to four clients.

CLIENT-MANAGED ATTENDANT SERVICES (CMA)

A demonstration project in which clients with physical disabilities hire and supervise their own attendants to provide services as needed.

COMMUNITY BASED ALTERNATIVE SERVICES (CBA)

Medically intensive, community based services for persons with medical necessity and chose the waiver as a cost-effective alternative to institutional placement. Services include 24-hour supervision, aides, emergency services, home modification, medical supplies, nursing, personal care, prescription drugs, rehabilitation therapies, and respite.

COMMUNITY LIVING ASSISTANCE AND SUPPORT SERVICES (CLA)

This Medicaid waiver program provides home and community-based services to people with developmental disabilities other than mental retardation. It is a cost effective alternative to ICF-MR Level VII institutional placement.

DAY ACTIVITY AND HEALTH SERVICES (DAHS)

Daytime care, five days a week in licensed centers. Patient services include exercise, hot meals, personal and nursing care, and social and recreational activities.

HOME DELIVERED MEALS (HDM)

A hot noon meal delivered to the home five days a week.

IN-HOME AND FAMILY SUPPORT SERVICES (IHFS)

Cash grants to persons with disabilities to purchase services, supplies and equipment to enable them to continue to remain in their home.

PRIMARY HOME CARE (PHC) OR FAMILY CARE (FC)

Non-technical, non-skilled services performed at home by an attendant to assist with personal care tasks such as bathing, dressing, grooming, toileting, ambulating and meal preparation. Home management tasks such as cleaning, laundry and shopping may be included. Escort to medical treatment may also be authorized.

Concord Primary Care Services, Inc. in collaboration with the Texas Department of Aging and Disability Services can provide preliminary screening and referral for the following services to the aged and disabled in the community.

NURSING FACILITY SERVICES

24-hour nursing care in a licensed facility.

RESIDENTIAL CARE

Services to clients who need access to care on a 24-hour basis but do not need daily nursing intervention. Services can include protective supervision, personal care, social and recreational activities, housekeeping and transportation.

RESPITE CARE

Short-term services to allow temporary relief to regular caregivers. Services can be provided in the home, nursing home, hospital or day activity center.
